Orchard Supply Hardware "Advisory Panel $2500 Shopping Spree"
SWEEPSTAKES 2009 OFFICIAL RULES
1. NO PURCHASE NECESSARY. VOID WHERE PROHIBITED. MAKING A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
2. ELIGIBILITY: Open to legal California residents, 18 years of age and older. Employees of Orchard Supply Hardware LLC (OSH), its respective parent companies, subsidiaries, affiliates, related companies and advertising and promotion agencies and the immediate families (parent, child, sibling and spouse of each) and members of the households of each are NOT eligible to enter.
3. HOW TO ENTER: Complete an OSH panel survey(s) during the following week(s): October 1-10, 2009; January 1-10, 2010; April 1-10, 2010 and July 1-10, 2010. Limit of (1) survey per period and (4) four surveys per participant. Each completed survey will account for (1) one entry in the Survey Panel Prize drawing and (1) entry in the Annual Grand Prize drawing. OSH reserves the right to modify the survey periods if required. All Advisory Panel participants will receive notification of changes to survey periods. The surveys become the property of OSH and will be used for business research. Sponsor is not responsible for lost, late, illegible, mutilated, misdirected, or incomplete entries.
4. DRAWINGS: Winners will be selected in random drawing from among all eligible entries received for one (1) of the following available prize(s). A random drawing for the Survey Panel Prize will be conducted following each survey on: November 15, 2009; February 15, 2010; May 15, 2010 and August 15, 2010. Total of five (5) winners will be selected in each random drawing. Four (4) winners will be selected for a $100 OSH gift card and one (1) winner will be selected for a $250 OSH gift card.
In addition, a total of (5) winner will be selected at a random drawing on August 15, 2010 among all entries received from the four Advisory Panel surveys. Of these, four (4) Winners will be selected for a First Place Prize of a $1000 OSH Shopping Spree and one (1) winner will be selected for a Grand Prize of a OSH $2500 Shopping Spree.
Winners will be notified via email. If prize notification is returned as undeliverable or if the winner is unreachable within 21 days, an alternate winner may be selected. Winners will be required to sign and return an Affidavit of Eligibility/Release of Liability and Publicity (where permitted) within 7 days following notification or prize may be forfeited and an alternate winner may be selected.
5. PRIZES:
One (1) Annual Grand Prize $2500 OSH Shopping Spree
Four (4) First Place Prizes $1000 OSH Shopping Spree each
Four (4) Quarterly Survey Panel Prizes
- Four (4) $250 OSH Gift Card (one per quarter)
- Sixteen (16) $100 OSH Gift Card (four per quarter)
Total Value of all Prizes = $9,100.00.
Limit one prize per household. Odds of winning depend on number of eligible entries received prior to the drawing.
6. GENERAL: Prizes are not transferable. Taxes on prizes are the sole responsibility of winners. No prize substitution or cash redemption allowed except by OSH, who may substitute a Prize of equal or greater value if advertised Prize becomes unavailable. By participating, entrants agree to be bound by these official rules and decisions of the judges. By accepting prizes, winners consent to use of names and likenesses for advertising purposes worldwide without limitation and without additional compensation, except where prohibited by law. By accepting prize, winners further agree to release OSH, and its parent companies, subsidiaries, affiliates, related companies and advertising and promotion agencies, and each of their respective officers, directors, employees and agents from any and all liability, loss or damage of any kind arising out of or in connection with their participation in this sweepstakes or with respect to awarding, receipt, possession, use and or misuse of any prize. All federal, state and local laws and regulations apply. The Shopping Spree is defined as a one-hour shopping period at an OSH store mutually agreeable to winner and OSH, and at a date and time designated by OSH, where the winner will have the opportunity to purchase as many items as possible, up to the prize limit in retail value. Any unused portion of the prize not spent within the one-hour Shopping Spree will go unused and will not be awarded. Winners must be available to participate in the OSH Shopping Spree on the date specified by OSH in order for prize to be awarded or an alternate winner will be selected.
7. WINNERS LIST: For a list of winners, available after September 15, 2010 and before October 15, 2010 send a self-addressed, stamped envelope to: Orchard Supply Hardware, Advisory Panel ,6450 Via Del Oro, San Jose, CA 95119.
